Your ACPHS Application

This ACPHS application is appropriate for the following students: 

• Prospective undergraduate students interested in completing a bachelor’s degree or pursuing admission in Pharmacy Early Assurance program. 
• Prospective graduate students interested in pursuing a master's degree. 

*Students that have already completed or are on track to complete prerequisite coursework and may be eligible for admission to the professional sequence of the Doctor of Pharmacy program should review the separate application process and requirements on this site.
